Mine was the head of the Scorpion clan on my OA campaign, that my PCs killed as Dofuku ( giant spider ). I would love to use an ex PC as an ex-paladin or Lord Soth if I throw them in Krynn.

I love reoccurring villains, I try to have one or two in every campaign I run.

Currently I have 3 going on in the RHoD one was meant to be reoccurring and two just worked out that way.

The 'not so sick spy' Miha Serani has shown up 3 times to pump the party for information and been in the background at some important meetings. SHe has evaded or escaped the party each time and will return for the final battle.

Regiarix surrived his battle in Rhest and has returned twice to harry the party both times barely escaping with his life but disrupting the partys plans in the proccess.

And Nurklenak the pretty much generic mindbender also managed to charm and control the party to escape and has been confounding the group at almost every turn, to the point my players made a 'list' of NPC's that needed killing and he was at the top . He was so annoying with his constant hold persons,suggestions and charms that the last time he appeared,during the streets of blood, the party actually stopped fighting the other creatures to concentrate on taking him out.

Ethandrul was an elven necromancer who started as my pc. He had a kind of endearing creepiness- he fawned over a female pc- but never enough to bring much notice  from her. eventually though, he grew to hate the party. He made the entire party earrings of spider climbing- that contained a hollow bead full of dragon bile ( iirc) poison. then each one was scribed with an exploding rune. - Our Dm was geting sick of his job so I offered to swap with him (an arrangement made before the "gifts" were given) In the middle of a huge battle with a pair of Golems I saw my chance- 3 of 5 party members were on the vaulted ceiling- attacking from safety. Ethandrul decided to loot the room while the party was engaged.- a stray arrow nearly hit him- so he spoke a command word- and all of the earrings exploded- 5d6 damage to 3 heads...save vs. poison...5d6 from fall. hehehe. He did lots of good stuff like that.

A current one in my campaign is a Large Magical Owl.  It's not evil, but it really wants to get the party alone, or the horses alone to eat them.

The druid had to speak with it, and the owl was so intrigued by the thought of food that talks he invited her up into his tree to have a "conversation".  Also every time they went into a nearby building the owl would show up on the roof or land next to the horse or just generally terroize the party.

The player who is playing the knight is constatnly doing search checks for the owl.  The druid is going to get a new animal companion soon, I'm giving it a 5% chance that the owl will answer the call just to piss the party off.

A giant owl stalking the PCs? Now, THAT's classic :D

I have two recurring "villains" that pop up now and then. The first is a halfling rogue named "Neville." It was a character from one of my first campaigns that I DMed. Neville wound up literally owning the rest of the party members due to the fact that he'd hoarde treasure then make loans to the rest of the party. Humorous, but disruptive. Neville shows up occasionally to terrorize new PCs with his low-rates and long-term leases on their souls (he works for a glabrezu so can negotiate whatever a PC might 'wish' for).

The other 'villain' that pops up now and then is Gulthias the vampire (WotC 3.0 Adventure Path). I've run that adventure path a few times and no one has yet been able to off the sunglasses-wearing vamp.

Most villains tend to make more than one appearance in my campaigns. Early on they'll leave their minions to "take care of the light work" or they'll be encountered in a role-play setting. Later on they might use teleport magic, minions, secret passages, flight, etc. to escape one the chips are down, just to pop up again after taking time to rest and regroup.

I have a couple:

Koritha aka The Blessed. She's a Tiefling Sorceress/Acolyte of the Skin. She hasn't attempted to kill the PC's, but she's done plenty to tick them off. Plus she's a real B**CH. She has alternative Tiefling abilities created by Monte Cook that are conversions from the old 2e Planescape Planewalkers Handbook. This gives her enough abilities to make her extremely annoying to the party. She started off as their villain when she threw down a rolled up soiled cloth infront of the party sorcereress's feet and then jumped a portal. The PC picked up the cloth, realized it was bloody, unrolled it, and found the skinned remains of her fathers face.

Then there's a new guy I'm trying to make into a reocurring villain:

Nabaelba, a Gnome Druid/Vermin Lord. The parties Druid died while adventuring in Baator. So the PC's buried him literally in Hell. So I created a scenario where they buried the body in this Gnomes Grove. The PC Druids Wolf animal companion ran off after his death. When the party met the ticked off Gnome, Nabaelba had a new Wolf Animal companion....except his time in hell turned him into a raving mad wolf with bloody fur. Nabaelba also has 2 Fire Beetle pets named Naba and Elba.
