Our House Rules:

0-The DM never buys the Pizza.
1-The DM is always right even when the rules explicitly say otherwise.
2-Monster's are not identified by the DM, only described.
3-1st level characters always begin with Max Hps.
4-Pc's can be brought to -5 health and still be unconscious. -6 is dead, and must be resurrected.
5-Full night of sleeping with food equals +3d4 health. On watch person gets +1d4 health.

Player issues aside this is precisely the reason I loath critical fumble charts. Exactly how many combats end with a person cutting their own leg off, irl? Stuff like that shatters any suspension of disbelief I may have and reminds me that I am playing a game (with some pretty silly rules).

Well if I remember correctly, he rolled really bad. Like a 99 on a 1d100.
So it was a freak accident type thing.
And it wasn't like we were rolling the dice for him.
The great thing about the Critical charts, is the really cool criticals you can get on other creatures/NPCs.
We all agreed to use it, for better or worse.
It is when you roll really bad, that it stinks, and he rolled about as bad as you could. ( I think he lost 2 points of dex too, from the mobility problems )

Anyway...
I haven't used the chart since that episode 10 years ago.
Instead I made up my own chart that is not nearly as brutal. With more things like being stunned or knocked unconscious. Taking double damage from your own weapon, is about as bad as it gets. No more limb loss. To pshycologically damaging...

We combine Open locks into disable device.
Vorpal weapons only exist in the hands of outsiders that you cant get!
Critical successes. if you roll a 20 on a skill check its not an auto success but you get to roll another 20 and add it to it (these can stack as the 3 20's in a row diplomacy check happened once)
Critical failures. If you roll a 1 on a skill check you minus 10 off whatever your roll is.

Some good ideas here...

We similarly use the escalating critical success or failure on skill checks (1 or 20 causes a re-roll with a +20 or -20 modifier to it). But I'm thinking the -10 might be a better idea.

Also use escalating 20's in combat. A 20 is a hit and a critical threat, a 2nd 20 is a critical and threat for instant death. A third roll can confirm instant kill. (Haven't actually had any monsters roll that yet, but I'd be inclined to simply reduce the PC to -5 or so -- taking him out of the fight bu not outright killing him.) Also, they have to be natural 20's -- no expanded critical threat range helps here. Every once in a while a roll like that happens, and the players get all excited about it and talk about it for sessions. (Remember when he took down the frost giant in one might hit? Wow!) However, while rereading this post, I may modify it to force a Fortitude save against massive damage or something similar, if it starts to get out of hand.

On critical misses (1) when I DM I usually roll a percentile dice and if less than 10%, something happens (usually drop weapon, trip, break bow string) depending on what seems appropriate. Generally the result is only the loss of an action (rolled a 1? Lose a turn!).

My friend tried using the double 20 is insta death double 1 is insta death... it got silly so we dropped it.

OK, as I've said, I don't know that critical fumble chart, I created my own. Here's an example, my slashing + piercing melee weapons chart.

When you roll a natural one to hit on your first (and only first) attack in a routine, roll again, adding your BAB, enchancement bonus for weapon, and any to hit bonuses from weapon focus or the like. If this result is under 20, roll percentiles on the critical miss chart.

1-20 Overswing! -2 AC next round.
21-35 SKRANG! You hit something hard, you are dazed next round.
36-50 Bad overswing! Your target may make an attack of opportunity against you.
51-60 Wild overswing! You are flat footed until your next round.
61-75 Whoops! Drop your weapon.
76-85 Shaky footing! Fall prone.
86-95 Crunch! Embed your weapon in a wall or floor, make a DC 12 strength check as a standard action to free it.
96-99 Ouch! Hit yourself, this is automatic and cannot critical, unless the failure confirmation roll was also a natural 1 (treat as a critical threat).
100 CRASH! Resolve a sunder attempt against the weapon, treated as though a mundane version of the same weapon were being used to make the sunder. Note that this ignores the rules for damage types.
