It's actually the management.
If you're management style is broken, and you've recognized that, why would you continue to hire more managers (or promote) people with the exact same style? Maddening.
Furthermore, the latest promotion was basically handed to a guy who has no idea how to do it, but based solely on how much he is liked. Grrr...
